About the Social Work Department

The Social Work Department provides Social Work services across acute and sub-acute inpatient wards (some differences across sites) in a range of areas including general medicine, general surgery, Women’s and Children’s, Emergency, Intensive Care, GEM and Rehabilitation. A weekend service and on-call service is also provided to the acute wards across the bed-based sites.

About the Role

These positions will provide the successful incumbents with the opportunity to work across numerous Monash Health sites which include Monash Medical Centre, the Kingston Centre, Dandenong Hospital and Casey Hospital and to actively participate in the Allied Health New Graduate Program.

There are four full-time positions (fixed term – February 2022- December 2023) which are rotational across Monash Health acute and sub-acute services. The positions will suit social workers who are well organised and resourceful with skills in psycho-social assessments, problem solving, communication and hospital discharge planning.  These unique opportunities will provide outstanding professional development and clinical experience across acute and sub-acute healthcare settings.

You will be a member of a dynamic Social Work and Allied Health team working in a collaborative model of care across Monash Health providing:

  • Psychosocial assessments, risk assessments, patient and family sensitive model of care

  • Discharge planning for complex caseloads including My Aged Care and NDIS transition

  • Team approach with the broader multi-disciplinary team
