About the Role

As the successful applicant you will contribute to and be part of the community based multidisciplinary health team to support consumers and carers to achieve an optimum level of health independence through:-

  • Psychosocial assessment involving the patient, family and engaged agencies
  • Provide clinical intervention ensuring that the needs of the client and family are attended to.
  • Work alongside team, providing consultation where required.
  • Refer patients to appropriate Barwon Health departments or external agency as appropriate.
  • Maintain appropriate records as required by Barwon Health and statutory authority
