An exciting opportunity exists for a passionate and experienced Social Worker to join the Western Health at Home team in the GEM@HOME service.

GEM@HOME provides an alternative option to hospital based rehabilitation by offering holistic multi-disciplinary GEM-type care for older people within the comfort of their home rather than in hospital.

GEM@HOME aims to:

• Improve the patient experience by providing care in the comfort of their own home


• Assist patients to achieve their care goals and best possible level of health and function


• Promote and support safe and early discharge from hospital settings

The successful applicant will work in partnership with a strongly committed team to provide high quality and efficient care and support to patients in the home environment.

The GEM@HOME team is looking for candidates that:

• Demonstrate exceptional interpersonal and collaborative teamwork skills


• Display high level clinical reasoning skills


• Will be proactive in supporting discharge to the community

To be successful in the role you must have:

• Bachelor/Masters of Social Work degree and eligibility for AASW membership


• Minimum of 3 years clinical experience in public health


• Relevant hospital and community related experience


• Highly developed interpersonal skills and communication skills


• Demonstrated ability to work collaboratively in multi-disciplinary teams


• A current Victorian drivers licence
