About the Social Work Department


The Social Work Departments provide Social Work services across acute and sub-acute inpatient wards (some differences across sites) in a range of areas including general medicine, general surgery, Women’s and Children’s, Emergency, Intensive Care, GEM and Rehabilitation. A weekend service and weekend on call service is also provided to the acute wards across the bed-based sites


About the Role


Monash Health Social Work Department is looking for an experienced Grade 3 social worker with high level experience in the adult sub-acute setting to join the team. The position is a permanent full-time position based at the Kingston Centre


The purpose of the Team Leader position in Social Work is to provide leadership and supervision in a professional, high quality, patient and family focussed approach. You will providing leadership and clinical care primarily across the sub-acute stream in both Rehabilitation and GEM (Geriatric Evaluation & Management).


The role requires you to have high level clinical skills working with patients with a variety of complex health and rehabilitation needs requiring expert clinical leadership that ensures safe, high quality Social Work service delivery. You will have competency in risk assessments, care planning, decision-making and discharge planning as well as liaison with internal and external key stakeholders including statutory services and senior hospital staff.


You will be a member of a dynamic team applying evidence based, informed, client centred care within a multi-disciplinary setting.


What You Need:



  • An Australian Association of Social Workers (AASW) accredited Bachelor Degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the AASW.

  • Written evidence of eligibility for AASW membership for overseas qualifications is required.

  • Minimum of seven years’ experience as a qualified Social Worker

  • Relevant clinical and leadership experience in the provision of Social Work, preferably in health care, with supervisory and service co-ordination experience

  • Current Working With Children’s (WWCC) check
