Social Worker - Hospital Admission Risk Program (HARP)

  • Permanent, Full Time position with monthly ADO (part time applications will be considered)
  • Grade 2 - $83,361.20 - $95,825.60 p.a. (dependent on experience)

About the role

Eastern Health HARP provided short term care coordination to clients in our catchment. The Grade 2 social work role involves assessments of clients within a multidisciplinary team, providing high level consultation to other professional staff and participating iin day to day team processes.

About you

To be successful in this role you will possess:

  • Eligibility for membership of the AASW.
  • Evidence of current CPE, as per AASW requirements.
  • 3 years post graduate work experience
  • Highly developed interpersonal skills.
  • A strong commitment to high quality care.
  • The ability to work effectively as part of a diverse team and also work autonomously as required.
  • Excellent communication and organisational skills.
  • Excellent risk assessment skills.
  • The ability to lead complex discharge planning.
  • Conflict resolution skills.
  • Clinical experience in relevant community sectors.
  • Knowledge and understanding of a broad range of physical and psychological health conditions and their potential impacts.
  • Consumer focus and motivational interviewing skills and experience.
  • Current Victorian Driver's Licence
  • Computer literacy
