Welfare Worker (XM9-XM15)
Permanent Full Time
(80 hrs per fortnight) with ADO

The Position

Applications are invited from skilled and enthusiastic individuals to join our fast growing Community Options team. This permanent full time position is available for an immediate start and includes working Monday to Friday. The successful applicant will work closely with clients, their families and service providers to provide a variety of choices most suited to their care needs.

The Department

Community Options is part of Wimmera Health Care Group and is located in modern offices in central Horsham, opposite May Park. They are an expanding business that supports aged community members to remain living in their homes for as long as possible. They also support those under 65 who have a chronic or palliative condition not qualifying for assistance under the National Disability Insurance Scheme. This is achieved with support of Home Care Packages and Linkages programs funded by the Commonwealth and State Governments, administered by qualified and skilled team members.

Key Selection Criteria include:


  • Relevant qualification / experience in welfare, social work, nursing or allied health

  • Advanced communication and interpersonal skills

  • Excellent computer and documentation/reporting skills and willingness to learn required IT programs

  • Demonstrated commitment to quality service to consumers

  • Current Victorian Drivers Licence



  • Demonstrated understanding of and skills in case management.

  • Recent experience with community based services for people and their carer’s

  • Demonstrated skills with advocacy, negotiation and conflict resolution

  • Demonstrated ability to work effectively within a multidisciplinary team
