What You Will Be Doing

The Transition Care Program (TCP) is seeking an experienced, innovative and flexible individual to work as a member of the TCP Team. The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long-term care arrangements.

TCP operates in both the Community as well as two Residential Aged Care environments, offering scope for clinicians to enhance their experience along the care continuum.

What You Need

We anticipate you will have:
• Eligibility/Membership of relevant professional body
• Experience in a sub-acute and/or community setting
• Demonstrated competence in a specific RN or Allied Health area of practice
• Knowledge of / experience in Aged/Residential Care settings.
• Current Driver’s Licence
