Sub-acute Care Coordinator

ABOUT THE ROLE: 

Werribee Mercy Hospital is a public hospital, servicing the growing community of the West. Our inpatient subacute ward delivers care to a diverse range of patients who require complex interdisciplinary interventions after a referral from a variety of settings. We have an exciting opportunity for an experienced Registered Nurse to join our team as a Subacute Care Coordinator. Working in collaboration with our inpatient ward Nurse Unit Manager, this role is responsible for the effective management, coordination and care planning of patient presentations via the following pathways:

  • Acute to Subacute (inpatient)
  • Transitional Care Program (TCP)
  • Acute to GEM@home
  • Subacute to @homeacute

As the Subacute Care Coordinator you will: 

  • Facilitate early referrals and coordinator care for patients who are identified as suitable for a subacute treatment pathway
  • Assist to facilitate access and flow for admission into subacute streams for suitable patients
  • Improve clinical support for subacute staff
  • Work with stakeholders to manage risk and identify deterioration
  • Foster collaborative relationships with key stakeholders to develop coordinated management plans

 ABOUT YOU: 

To succeed in this role, you will have a minimum of 5 years experience with at least 2 years experience in subacute nursing in the hospital setting, as well as;

  • Excellent communication skills
  • Ability to engage with key stakeholders and be a driver for change
  • Demonstrate strong clinical assessment and problem solving skills
  • Advocate for the best clinical outcomes and delivery of patient centred care
  • Lead and inspire a team of clinical professionals to ensure a positive patient experience
  • Work within the values of Mercy Health
