What You Will Be Doing

Peninsula Health is seeking an experienced, innovative and flexible Case Manager (CM) to work as a member of the Transition Care Team.

The Transition Care Program is a jointly funded DHS/Commonwealth Program designed to assist elderly patients transfer from their ‘inpatient hospital stay’ either to their home or to residential care. The aims of the program are to assist these patients attain their maximal functioning capacity and to help determine their long term care arrangements.

What You Need

The successful applicant will need to:
• Demonstrate highly developed clinical assessment, therapeutic discharge and care planning skills
• Demonstrated knowledge of issues affecting patients with complex care needs
• Demonstrate highly developed knowledge and understanding of Residential Care and the Community Service sector and their relationship with the health service
• Flexibility to work across a number of different areas from Emergency, to Acute and Sub-Acute Inpatient services.

The CM should be capable of managing a range of clinical conditions and social circumstances.

The successful applicant will need to be highly motivated, innovative and enthusiastic and to be able to work both independently and as part of a team.

This is a dynamic, challenging and rewarding position, suited to a CM with a commitment to Best Practice in Integrated and Multi-disciplinary Care.

Experience in one or more aspects of Health or Community Care is essential.
