THE ROLE

The Team Leader, Care Services Support Packages leads the implementation, coordination and monitoring of activities associated with individualised support packages through programs such as Targeted Care Packages and Home Stretch across East Division.  It is a critical role within Care Services Operations Support to deliver improved outcomes for children and young people in care services and supporting their transition from care to independent living.

The role is responsible for managing the team and its operations, demonstrating strong planning, coordination, and stakeholder relationship skills to manage and design, implement, and monitor tailored supports to meet the needs of individual children, their families and carers, providing flexible and innovative solutions for children and young people.

ACCOUNTABILITIES INCLUDE

  1. Undertake and lead activity in relation to the rollout and design of individualised funding packages used to support children and young people in care services.
  2. Administer the use of all funds associated with individualised and targeted packages.
  3. Ensure review and quality mechanisms are in place and that all packages are monitored in accordance with departmental requirements.
