Position Title: Intensive Care – Transitional Fellow

2025 Training Year commencing February 2025

Unit Description:

Royal Melbourne Hospital Intensive Care Unit is a state-of-the art 42-bed facility providing care for almost 3000 patients per year and is one of the biggest and busiest ICUs in Australia. We are a level 3 adult unit caring for general and specialty medical and surgical patients, including neuro- and cardiac surgery. We are also an intermediate ECMO centre, one of two major trauma centres in the state, and the also provide intensive care services for patients at the Royal Women’s Hospital, Peter MacCallum Cancer Centre and critically unwell patients from rural, regional Victoria and interstate.

The Intensive Care Unit is accredited for 2 years of core training by the College of Intensive Care Medicine including cardiac and neuro ICU and is one of only two Victorian sites accredited for trauma sub-speciality exposure. We offer a highly successful teaching program for ICU trainees, with our consultant team actively involved in the delivery of registrar and resident teaching, including exam-focussed teaching for both Primary and Fellowship exams. The RMH ICU education team runs several courses such as MCEC, BASIC, REACT, CALS and critical care echocardiography. Many opportunities exist for trainees to undertake research and complete their CICM formal project with supervision from experienced consultant researchers.

The ICU has four pods with a staffing structure ensuring trainees are very well-supported. In hours, the Intensive Care Unit is staffed by four consultants, two senior registrars or transition fellows and four registrars, one for each pod, plus an external outreach consultant and registrar. Consultant support is available at all times out of hours. From 7.30 am to 12.00 midnight one HMO is rostered for each pod, with a single HMO rostered overnight. Registrars have 5 hours a week of protected and paid training time with a consultant-led education program delivered each week.

About the role:

This role is that of a Transition Fellow within the Intensive Care Department, located at Royal Melbourne Hospital.

This role is between that of a registrar and consultant, providing flexibility and graded responsibility to allow increasing clinical autonomy, plus development of skills in research, education, supervision and administration. It is aimed to assist the movement of the trainee into specialist practice.

The clinical role will comprise Senior Registrar and Transition Fellow shifts and will include on-call duties. Transition trainees will be assigned non-clinical hours (10%) plus 10 hours training time per fortnight.

Candidates who have/will have satisfactorily completed all the necessary CICM requirements to be eligible for entry into Transition Training by the time of their commencement are encouraged to apply. There are vacancies for positions in Research, Education, Quality and Safety and Critical Care Echocardiography commencing in February and August 2025 These roles have received prior approval from CICM.

Please note: Employment in the role of Transition Year Fellow at RMH ICU is dependent on acceptance into Transition Training by CICM but does not guarantee this. Successful applicants will need to obtain prospective approval from the College prior to commencement of transition training. All applicants are strongly encouraged to review CICM requirements for transition training prior to applying for this role.

About You:

  • Registration with the Medical Board of Australia (AHPRA) or eligible for registration with the Medical Board of Australia (for international Medical Graduates)
  • MBBS or equivalent tertiary qualifications.
  • Ability to work in a team including supervision of junior medical staff
  • Well-developed leadership, communication and presentation skills
  • Registration as a trainee with the College of Intensive Care Medicine, with successful completion of the second part examination
  • Extensive experience in intensive care medicine, with broad case-mix exposure
  • Completion of all CICM training prerequisites prior to commencement of transition training, including:
    • Prior experience in at least two out of three subspecialities of cardiothoracic, neurological/neurosurgical and trauma intensive care medicine.
    • Submission of CICM formal project
    • At least 12 months each of anaesthesia and medicine training
    • All other required courses and learning
