About the facility 

Barwon Health's Mental health, Drugs and Alcohol Services (MHDAS) provides integrated and recovery orientated mental health and drug and alcohol assessment, treatment, harm reduction and community and family support to people of all ages within the Barwon Region currently experiencing, or at risk of experiencing mental health and/or drug and alcohol issues. Our service is internationally recognised for its innovative clinical programs and integrated models of practice that are consumer centred, accessible and family inclusive. We are fortunate to enjoy excellent internal relationships and have developed excellent partnerships with local community that provide for seamless integrated care for our clients.

About the role 

Aged Psychiatry Service prides itself on delivering high quality client-centred mental health care for people aged 65 years and above, and their families/carer.

This role includes working within a multidisciplinary team that provides a range of psychiatric case management services to Aged Psychiatry clients, their carers and families within a community setting in the Barwon Region. This may consist of nursing home, residential service, their own home and/or the carer’s home. Clinicians provide comprehensive assessments, crisis intervention, outreach case management, formulate and review Care Plans in collaboration with the client and other parties concerned with the client’s care and refer to other service providers when appropriate.
