Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.

NHW have an exciting opportunity for an experienced Registered Nurse or Allied Health Professional with a passion for sub-acute care to join our Transition Care Program team as a Care Coordinator to provide comprehensive clinical assessment, care planning and service coordination to our clients. The successful applicant will join a well-established multi-disciplinary team working closely with clients to maximizing functional capacity, social participation and self-management after hospitalisation.
