Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.


In conjunction with the Department of Health, Austin's Mental Health division has created a new and exciting Intensive Community Care Team. This multidisciplinary team has been developed to meet the ongoing needs for one of our communities most complex and vulnerable cohort of clients. This team will work in conjunction with pre-existing community teams, to develop strong therapeutic relationships with their clients, in order to better identify and meet rehabilitation goals through use of best evidenced-based frameworks and assertive case management.


A new and exciting opportunity currently exists for an enthusiastic and highly experienced Mental Health Practitioner to join the North East Area Mental Health Service (NEAMHS).


The Intensive Community Care Team will work closely with current NEAMHS community teams, such as the North East Continuing Care Service (NE CCS) and the North East Mobile Support Treatment Service (NE MSTS).
Hours of operation will be Monday - Friday 8:30am - 5:00pm.


The Intensive Community Care Team will have a multidisciplinary approach and will have the capability to deliver specialist clinical care for clients with severe or complex mental health diagnoses. The clinicians working within this team will undertake a clinical case load as per the teams requirements to provide intensive clinical interventions with targeted goals unique to the individual.


To be successful in this role you must demonstrate key selection criteria, some of which include:


Mandatory Qualifications - relevant professional qualification in a health related discipline (psychology, nursing, social work, occupational therapy)


A minimum 3 years of experience in a relevant mental health setting.
Demonstrated knowledge of recovery and collaborative clinical practice.
A thorough working knowledge of the Mental Health Act (2014), the Carer Recognition Ace (2012), and Chief Psychiatrists Guidelines
