Community Rehabilitation


The Community Rehabilitation offers both home and centre based therapy. The centre based program   offers rehabilitation services to clients within the context of the rehabilitation gym and community. The case manager works closely with the interdisciplinary community rehabilitation team to provide a therapy program that is both holistic and client centred. This position provides the opportunity to work with autonomy, be creative and deliver high quality therapy services.


Job Summary



  • 72 hours a fortnight


The Case Manager is a core part of the multidisciplinary team and will actively contribute and



  • Work collaboratively with clients to undertake a comprehensive assessment.

  • Provide client advocacy within the timeframes of the program ensuring long term sustainability of care plans through facilitation of self management skills and strategies.

  • Identify and manage carer support needs as part of the focus on sustainability of care planning.

  • Act as a resource person for other team members with Community Rehabilitation and across the Health Independence Programs.

  • Assist with client flow with in the organisation

  • Provide direct client care within the scope of practice of his or her professional background.

  • Contribute to achieving client goals as member of the Community Rehabilitation team.


What you need:



  • Bachelor level qualification in an Allied Health discipline.

  • Current registration with AHPRA (where applicable)

  • Eligible for membership of relevant Professional body

  • Current Victorian Drivers Licence

  • Working with Children’s check

  • Experience in community rehabilitation (desirable)
