Case Manager
Home Care Packages and Linkages
Multiple Positions available – both Full-time and Part-time
Fixed Term until 30 June 2025

About the role
Grampians Health is a local, trusted provider of Home Care Packages and Linkages Packages assisting our Consumers to remain living at home safely and with confidence. Services are delivered by our experienced and professional staff to promote functional independence, wellness and enhance individuals’ quality of life.

Applications are invited from professionals with appropriate tertiary qualifications (e.g. social work, rural social welfare, nursing, allied health) to provide case management in the Central Highlands region of Victoria. The Case Manager role includes maintaining a caseload of consumers and supporting them and their carers via effective goal planning, service coordination, advocacy and ongoing assessment within a Consumer Directed Care framework.

The successful applicant will be provided with appropriate orientation, supervision, ongoing support and professional development opportunities.
