About the facility 

The Hospital Admission Risk Program (HARP) is a community based program employing more than 30 staff, including nurses, physiotherapists, social workers, occupational therapists, psychologists and administration staff that provide outreach support for people with chronic health and psychosocial needs. HARP aims to decrease hospital demand through the provision of comprehensive assessment, care planning, care coordination and faciliating timely access to specialist care, for people who present frequently or are at imminent risk of presenting to hospital.

About the role 

HARP are looking for an experienced nursing or allied health clinician to fill a temporary full time position, until 26th June 2022.

The successful candidate will provide comprehensive assessment and care coordinator for a case load of clients with chronic health and/or psychosocial needs. All HARP Community Clinicians are expeted to work collaboratively with the client, their HARP colleagues and internal and external service providers to work towards improved health and wellbeing outcomes, whilst supporting clients to work towards actively managing their own health needs.
