Community Partnerships Unit/GP Liaison Coordinator



  • Work at the interface between the RMH and General Practice

The Community Partnerships/GP Liaison Unit work closely with teams across the Royal Melbourne Hospital to improve communication with General Practice.  We have strong relationships directly with General Practice and via the North Western Melbourne Primary Health Network.


The Unit provides the primary care perspective to RMH Committees, working groups and projects and helps ensure that the patient is at the centre of care along the journey through the hospital back to their community.



Your new role:



  • Be the first point of contact for GP enquiries

  • Monitor outgoing communication systems to General Practice and other external providers

  • Work closely with GP Liaison colleagues within the Parkville precinct hospitals and clinical staff at the RMH

  • Initiate and participate in improvement activities involving the Acute/primary care interface


About you:



  • Experience and understanding of a large health service

  • An understanding of the current General Practice environment

  • Great communication and problem solving skills

  • Ability to negotiate, engage and think outside the square (Be creative!)

  • Initiative and the ability to work autonomously
