Our Journey:

Monash Children’s at Home (MC@Home) provides healthcare services for infants, children and their families at home and in the community, who require ongoing specialised and often complex medical and nursing care.

We currently have a number of opportunities within MC@Home for highly motivated professionals to join our dedicated paediatric/neonatal HITH/PAC nursing team.  We have Full and Part Time ongoing roles working across a rotating seven day roster.

A Day in the Life:

Reporting to the Nurse Unit Manager, you will work largely independently, providing a safe, effective alternative model of care for Newborn/Paediatric patients needing ongoing nursing management at home.  In this way, patients spend less time in the acute setting and we concurrently uphold our commitment to patient centred care.

• Conducting comprehensive health assessments and educating those involved in care planning, direct care and discharge planning in the ambulatory setting


• Ensuring timely discharge planning to facilitate patients’ safe and effective transition from hospital to home


• Working across a multidisciplinary team, you will  you ensure that patients enjoy a safe transition from the acute hospital setting to the home environment

What You Bring:

• The ability to plan, evaluate and maintain a high standard of safe and effective care for children


• Strong clinical knowledge allowing you to work autonomously and make sound clinical decisions for your patients


• Proven ability to apply excellent clinical and critical thinking skills while working independently but within a Team framework

On a Personal Level, you:

• Will demonstrate your commitment to developing, promoting and progressing continuous quality improvement initiatives


• Are a dedicated nursing professional, and can demonstrate initiative and sound judgement in both decision-making and work planning


• Will role model excellence in safe nursing practice consistent with our expectations of leaders across Monash Health

What you need:

• Current AHPRA registration


• Current Acute Paediatric or Neonatal Nursing experience


• Demonstrated accountability for providing high quality nursing care


• Proven ability to apply excellent clinical and critical thinking skills while working independently but within a Team framework


• Excellent communication, interpersonal and documentation skills


• Current Driver’s Licence and access to a reliable vehicle


• High standard Computer skills
