Child and Family Program


The Child and Family Program including Healthy Mothers Healthy Babies (HMHB) is comprised of numerous disciplines that focus on assessment and  Child and Family focused care. The program consists of Counsellors, Occupational Therapists , Speech Pathologists, Dieticians, Podiatrists,Psychologists, Physiotherapists, Exercise Physiologist , Community Development Workers and Family Support Workers.  The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace  child and family centered practice, evidence based practice and the Social Model of Health.


Purpose


The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning.  Social Workers function as members of the multi-disciplinary team and have a pivotal role where social complexity exists.


The Grade 2 Social Worker will be an experienced and skilled clinician with consolidated psychosocial assessment and planning skills and able to use evidence to inform practice. They will also provide supervision of clinical staff, and students and contribute to service development. There is an expectation that effective, high quality care to patients across the health service will be provided.


Key result areas


The position requires the incumbent to work enthusiastically and flexibly in line with the AASW practice and the Monash Health Credentialing and Scope of Practice standards.


The incumbent will support Community Services to achieve targets, improve service efficiency, service coordination and the client experience. The incumbent will have an up-to-date knowledge of community health as well as an understanding of community health philosophy, programs and services. This includes a commitment to the social model of health and health promotion.


Further, the incumbent will, as a member of the Child and family Program, commit to providing high quality, Social Work, including assessment and treatment, to clients living in the City of Greater Dandenong, City of Casey and Shire of Cardinia.


As would be expected of all employees, the incumbent will be seen to positively contribute to the functioning of the team, to support innovation and contribute to the ongoing development of the service. The incumbent will be committed to the successful realization of the Monash Health Community model of care as well as Monash Health goals and values.


The Social Worker has a key role in supporting the service at all times.


The key service areas include:



  • assessment and planning;

  • crisis intervention and solution focused interventions;

  • referral to and liaison with community services,

  • active engagement with a range of internal and external key stakeholders and the multidisciplinary team


The nature of this work requires a flexible approach and the ability to adapt to a broad range of working environments.


The Social Work Service has a key role in assessment and management of:



  • vulnerable clients including family violence, elder abuse and infants/children at risk

  • making applications to the Victorian Civil and Administrative Tribunal  where appropriate for clients with a disability

  • comprehensive care planning and case co-ordination

  • referral to, and liaison with a range of community services

  • crisis intervention


 The Child Health Role within program



  • Advocacy for access to a broad range of service, such as the NDIS and other community services/providers

  • Crisis intervention

  • Family violence identification and management/referral to specialist support

  • Parenting support and information

  • Linkage to services

  • Assisting families to access services such as material aid and housing


Qualifications/ Registrations



  • AASW accredited Bachelor Degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers.

  • Written evidence of eligibility for AASW membership for overseas qualifications is required.

  • Minimum of two year’s experience as a qualified Social Worker

  • Working with Children’s Check

  • Australian Driving Licence

  • Current Police Check

  • Accredited Social Workers: As approved by the AASW (desirable)


Other position requirements:



  • As we support a culture of safety through employee immunisation, there must be documentation provided confirming completed immunisation on employment at Monash Health

  • Current and satisfactory Police Check (must also comply with Aged Care Act 1997 Accountability Amendment Principles 2012 if working in Aged Care setting)

  • Current and satisfactory Working with Children Check.

  • Applicants who are new to Monash Health will be required to enter into an Employment Agreement before commencement

  • Applicants who are new to Monash Health must provide evidence of immunisation before they can receive an offer of employment


In addition, the role requires excellent administrative, IT, organizational and communication skills, the ability to work independently, a willingness to work across Community Services sites as well as in an outreach capacity and a commitment to improving the way health services are delivered for patients. This position will work across multiple sites as service requires and will work collaboratively with a broad range of clinical teams and key stakeholders, within and external to the program.
