Grade 2 Physiotherapist - Early Stroke Discharge (ESD)

About the Role
The Early Supported Stroke Discharge (ESD) service provides community based multidisciplinary care to support patients transitioning from bed based services to their own homes.
The ESD service is split delivers care under both ambulatory and bed substitution models and provides rehabilitation services 6 days a week.
This is a Monday to Friday position mainly based at the Angliss Community Rehabilitation Centre Upper Ferntree Gully but will also on occasions cover Peter James Centre Burwood East.
