About the Role

The Early Supported Stroke Discharge (ESD) service provides community based multidisciplinary care to support patients transitioning from bed based services to their own homes.

The ESD service is split delivers care under both ambulatory and bed substitution models and provides rehabilitation services 6 days a week.

This is a Monday to Friday position mainly based at the Angliss Community Rehabilitation Centre Upper Ferntree Gully but will also on occasions cover Peter James Centre Burwood East.

About you

To be successful in this role you will possess

  • Experience in the area of neurological rehabilitation.
  • Be dynamic and enthusiastic grade 2 physiotherapist who is able to work a Monday to Friday roster
  •  You will be committed to high quality, evidence-based patient care and have the ability to work collaboratively and autonomously.
  • Advanced skills in the assessment and management of stroke rehabilitation, providing therapy both centre based and in client's homes.
  • Strong interpersonal and communication skills will enable you to thrive in this role.
