Post Acute Care Coordinator

Classification: Nurses & Midwives EA, CAPR 2 Level 1 PAC Nurse (YU15)

Hours per fortnight: 32

Status:  Permanent Part Time

Applications are sought from suitably qualified Registered Nurses to work in the IPAC at Echuca Regional Health.

Key responsibilities include:

• Provide safe and comprehensive patient focused care coordination
• Provide rapid response and facilitate early discharge from public hospitals by purchasing brokered services
• Provide appropriate short term goal directed care

Our team requires someone who has:

• Previous care coordination experience
• Demonstrated problem solving and decision making skill
• High level interpersonal skills
