West 2 Acute is a unique 30 bed ward that specialises in Acute General Medicine.
A Day in the Life:
We focus on complex discharge planning to provide patient centred care. Our unit has a strong team commitment to the discharge planning process to ensure a safe and successful discharge.
This position will give you the opportunity to work within the multi-disciplinary team to deliver a high standard of care and improve patient outcomes.
What You Bring:
You are a passionate and dedicated RN, able to work with the multi-disciplinary team confidently and collaboratively to deliver high standards of care and improve patient outcomes. Your experience lies in a medical ward, with sound experience in a surgical background. You are autonomous and work well with others. Your main goal being improving the patients experience while involving the team and their families in care planning to ensure safe, patient centred care at all times.
