Community Team 1C – Senior Community Mental Health Key Clinician
Community Team 1 (CT1) provide care to consumers living in the CBD and the Inner West of Melbourne. Additionally, they hold the homeless portfolio for Royal Melbourne Hospital Mental Health Service (RMH MHS). CT1 applies an Assertive Community Treatment (ACT) model of care, which is characterized by lower caseloads, community-based treatment (outreach) a team-based approach and high intensity of care. This can involve medication management, psychosocial support, assessment and referral (housing, financial, vocation, increasing levels of participation), psychological assessment, formulation and interventions, crisis assessments and admissions.
We are seeking an experienced mental health clinician to join and our 3rd clinical team we are forming at the RMH City Campus. The senior clinician will support the team to provide its strengths-based and personalised model of care, to increase consumers well-being and functioning and to support the delivery of evidenced-based psychological assessments, formulations and interventions.
This is a unique opportunity to work with complex consumers in a supportive team environment and to make a difference to some of the more vulnerable consumers in our community.
