Social Worker -Hospital Admission Risk Program

  • Permanent, Full Time position with monthly ADO (part time applications will be considered)
  • Grade 2 - $83,361.20 - $95,825.60 p.a. (dependent on experience)
  • Join our multi- disciplinary team providing excellent care to our clients in the community.
  • Position Supported by Grade 3 Social Worker within our community HARP team.

About the role

This is an exciting opportunity for a Social Worker to join our Hospital Admissions Risk Program(HARP) at Eastern Health. The HARP social worker will be supporting a complex cohort of clients in the community who have chronic diseases and psychosocial challenges. The HARP Social Worker utilises their knowledge, skills and clinical reasoning to conduct psychosocial assessments including risk assessments, as well as social work interventions for multiple complex issues. The HARP social worker requires a sound understanding of health service systems and provides secondary consultation to colleagues within the multi-disciplinary program in order to support specialised HARP team processes, working together to deliver best health care experiences and outcomes.


About you

To be successful in this role you will possess:

  • Eligibility for membership of the AASW.
  • Evidence of current CPE, as per AASW requirements.
  • 3 years post graduate work experience
  • Highly developed interpersonal skills.
  • A strong commitment to high quality care.
  • The ability to work effectively as part of a diverse team and also work autonomously as required.
  • Excellent communication and organisational skills.
  • Excellent risk assessment skills.
  • The ability to lead complex discharge planning.
  • Conflict resolution skills.
  • Knowledge and understanding of a broad range of physical and psychological health conditions and their potential impacts.
  • Consumer focus and motivational interviewing skills and experience.
  • Current Victorian Driver's Licence
  • Computer literacy
