About the role


 The Integrated Services- Hospital Admission Risk Program (HARP):



  • Aims to prevent avoidable hospital presentations and admissions by working with those clients at risk of, or already experiencing frequent emergency presentations or hospital admissions.

  • Targets people with chronic disease, aged and/or complex needs that frequently use hospitals or are at imminent risk of hospitalisation and could benefit from coordinated care.

  • Works in collaboration with other acute, community, aged care and specialist services to provide alternative interventions to hospital admission where appropriate.

  • Is part of the Health Independence Programs to provide the continuum of care for patients from Acute to community.


A HARP client is a person with chronic disease and or complex needs, who is frequently presenting to hospital or at risk of doing do. This includes:



  • People with chronic Disease

  • Older people with complex needs;

  • People with complex psychosocial needs; 


The Integrated Services Team  is responsible for coordinating care and teaching self management Skills to complex and chronic disease patients to limit presentation to ED or hospital if avoidable. They will also provide short term case management for complex Psychosocial issues identified in this group of patients.


This role is the professional oversight of the discipline within the  Integrated Services - Hospital Admission Risk Program in Eastern Health. The Grade 3 Senior Social Work Clinician is required to provide high quality care to clients and families through the autonomous use of expert clinical skills, highly effective communication and to provide professional clinical leadership to the Integrated Services Social work Team. You will demonstrate superior consolidated professional skills and knowledge in relevant area, a strong interest in quality improvement activities and research and the ability to develop allied health individuals and teams. You will work with other senior health professionals to guide the operational and professional performance of the  Team and ensure key performance indicators and targets are achieved. You will provide Professional and peer supervision to current social workers with assistance from the Director of Social work.


As part of the Integrated services  program, you will work closely with professionals from the multi-disciplinary team to ensure a holistic response to patients and to help achieve best outcomes  via support from a weekly case conference overseen by the clinical director within the program. This will support your development & career progression in the area of chronic and complex psychosocial work.


You will require strong skills in undertaking comprehensive psychosocial assessments, drawing on your clinical reasoning to formulate effective and individualised plans and interventions for patients, and must have a comprehensive understanding of relevant social work theory and its relationship to clinical practice. The role requires you to have highly developed interpersonal and assessment skills. A background in mental health will be highly regarded.

  • Post Graduate qualifications in Social worker or working towards

  • Highly developed interpersonal and leadership experience

  • A strong commitment to high quality care

  • Excellent communication and organisational skill  

  • Passionate about helping vulnerable clients
