An exciting opportunity exists for a suitably qualified Allied Health Professional to take the next step in their career and join the Rehabilitation Ward as a Subacute Intake Coordinator.
The Subacute Intake Coordinator collaborates with the Bed Manager, Rehab NUM and Rehab Clinical Health Lead to develop and maintain streamlined intake systems for the Rehab Ward in the context of the broader intake system for bed based services at ERH. The role also has overall responsibility for assessing all referrals and coordinating admissions to the Rehab Ward. The Rehabilitation Ward combines contemporary inter-professional practices and fantastic patient outcomes in a brand new state of the art rehabilitation unit.
The Rehabilitation Ward provides care for patients in four care types: Rehab, GEM, TCP and Palliative Care and therefore ensures a diverse and dynamic working environment.
