About the facility 


Hospital Admission Risk Program (HARP) is a community based program employing more than 30 staff, including nurses, physiotherapists, occupational therapists, dietitians, psychologists and administration staff that provide outreach support for people with chronic health and psychosocial needs. HARP aims to decrease hospital demand through the provision of comprehensive assessment, care planning, care coordination and facilitating timely access to specialist care, to patients who present frequently or are at imminent risk of presenting to hospital.


About the role 


As a HARP Team Leader, you will provide clinical leadership and support to a group of HARP Community Clinicians, in addition to line management, coordinating day to day work load, leave management, professional development reviews, accountability sessions and participate in and lead quality improvement initiatives.


The clinical component of the role is provision of direct client care to HARP clients.
